- Two SEALED letters of recommendation together with two Applicant Ability and Skills Rating Forms (One-School teacher or counselor/One-Community leader unrelated to applicant)
- High School Transcript (Applicants may submit a copy of unofficial or official)
- Verification of US Citizenship (If selected, copy of birth certificate is required)
- College Scholarship Application Certification Form
- Essay: In a minimum of 500 words, please address ONE of the following questions:
- What do you consider to be the most important societal problem? Why?
- Explain the importance of your planned major in today's society.
- What does it mean to give back to the community?
